Community Health Needs Assessment

Community Health Needs Assessments are conducted every three to four years in a process that collects information to identify the strengths, resources and needs of the County as a community. The information is then analyzed and compiled into a report that provides  information about the health of the community as it is today as well as the community's ability to improve the lives of the people who live here. The report is used by community members who come together to develop a plan of action to address the priorities identified through the assessment.

Live Well Wake

Formed out of the Community Health Needs Assessment and Population Health Task Force initiatives, Live Well Wake is a collaborative effort that relies on our community at-large to build partnerships to meet our goal. Through focused events and initiatives, we’ll aim to educate, motivate and learn about what our community needs to ensure collective community wellness.
